Referring Physicians Service

The Allergy Clinic central appointment patient services representative at 817-735-2583 is your primary contact for making patient appointments with Doctor Fling. In addition, the central appointment patient services representative will help answer your questions and work to resolve any concerns you may have about our services.
Our goal is to provide physicians and patients with prompt service and communication of complete results for clinic patients that we see. We strive to meet your requirements and needs for following patients throughout their care and will preserve the integrity of your relationship with your patient. If you ever refer a patient and are not completely satisfied with how we have served you, please let us know.

Scheduling an appointment:
To schedule patient appointments, call (817) 735-2583 between 8 a.m. and 5 p.m., Monday through Friday. You may also fax our appointment request/referral form if you would like us to contact you for an appointment date.

Our clinic is closed Saturdays, Sundays and holidays. However, our physicians are available for emergency consultation after hours by calling the central telephone number (817-735-2583).

You are always welcome to contact the clinic to discuss a specific patient or problem. Call the clinic telephone at 817-735-2583 who will take messages and contact the physician to assist in your needs.
Emergency Appointments:
Patients with urgent medical problems are given priority in our appointment system. Appointments are scheduled several days to several weeks in advance. We will, to the fullest extent of our resources, accept emergency cases on the same day. Patients requiring hospitalization or with emergency medical conditions should be referred to the closest emergency room or hospital for evaluation. Patients not requiring hospitalization should be referred to the Allergy Clinic for an outpatient appointment. We ask that all patient’s have appointments to be seen.

Date of Birth
SSN for patient identification
Marital Status
Type of Insurance
Primary Care Physician Name
Nature of Medical Problem
Consultative, evaluative, treatment services you would like provided.
Patient payment options:
We participate with most HMOs, PPOs and commercial insurance plans. We also accept Medicaid, Medicare and CHAMPUS. As a courtesy, we file claims on behalf of the patient.
If your insurance company requires a referral the patient and the patient’s primary care physician are responsible to ensure the referral is properly coordinated with the insurance company prior to the appointment date. Please check your referral request to make sure it covers the service requested, dates of coverage, and number of visits required. Our staff will assist you with any concerns that you may have.
