Membership Packages
***No one under the age of 16 is allowed in the Founders' Activity Center under any circumstances***

Faculty & Staff
Definition: Anyone verified by the Office of Human Resources as holding a faculty or staff appointment at UNT Health Science Center.
Fee: $10 per person per month
Documents required for sign-up: None
How Fees are Paid: Via UNTHSC Payroll Deduction or 1 year in full (cash or money order). If choosing payroll deduction, employees commit to at least 3 months of membership before termination can be accepted.

Visiting Scholars, Visiting Scientists, Visiting Students
Definition: Anyone who is designated as a "volunteer" on the UNT Health Science Center campus with Human Resources. Upon confirmation, visiting scientists, students, and scholars will receive a discounted membership rate of $20 per month to show our appreciation for the work they complete while at the UNT Health Science Center.
Fee: $20 per month*
*One-time $10 ID badge fee may apply during initial sign-up.
Documents required for sign-up: Driver's license or appropriate identification card.

Dependents of Employees
Definition: Individuals who are insurance eligible based on sponsoring employee's status. Not sure who qualifies as a dependent? Click here. **Limit three dependents per household.

3 months: $30*
6 months: $60*
9 months: $90*
12 months: $120*
*One-time $10 ID badge fee applies during initial membership sign-up.
Documents required for sign-up: Proof of address and current driver's license. Employee must be with member during sign-up.
How Fees are Paid: Members select one of the membership packages listed above and pay their fee directly to the FAC. All membership fees are paid via cash or money order only; Checks and debit/credit cards are not accepted.

Spouses and Significant Others of Students
Definition: Individuals wishing to obtain membership based on a relationship using this eligibility must have proof that he or she shares the same address with the student. **Limit one Spouse/Significant Other per household.

9 months: $90*
12 months: $120*
*One-time $10 ID badge fee applies during initial membership sign-up.
Documents required for sign-up: Proof of address that matches the student they claim relationship with and a current drivers license. Student must be with member during sign-up.

Alumni
Definition: Anyone verified with the Office of the Registrar as having received a degree from the UNT Health Science Center.

*One-time $10 ID badge fee applies during initial membership sign-up as old UNTSHC student ID badges are no longer valid after graduation.
Documents required for sign-up: Current drivers license.

Definition: Individuals, ages 16yrs or older, who are otherwise eligible for membership and have no affiliation with UNT Health Science center through academic, employee, or student standings.
