Medical Case Manager, Project Access Tarrant County-Catholic Charities of Fort Worth

In partnership with Tarrant County Medical Society, Project Access provides medical services to qualified patients in need.  As a medical case manager, I am an advocate for patients in the community with service providers and agencies.  My main responsibilities are monitoring and evaluating patient progress and adjusting service plans and/or delivery as necessary.   I also assist with the follow-up of pending cases by reviewing and connecting patients with specialists. 

Zeshan Chisty, MPH
CDC/CSTE Applied Epidemiology Fellow, Hawaii State Department of Health

This fellowship is designed to provide training and work experience in accordance with CSTE competencies.   I am working on a number of projects, from surveillance, outbreak investigations and public health prepardness projects. My main project is helping to maintain the State of Hawaii of Antimicrobial Resistance Project (SHARP). The data that is collected in SHARP contains the majority of the antimicrobial susceptibility test data in the state, and from this data we are able to track changes in antimicrobial susceptibilities. Currently I am looking at data from SHARP to determine the possibility of Carbapenem-resistant Klebsiella pneumoniae infections and the epidemiology of Methicillin-resistant Staphylococcus aureus in the state.

University of Alabama Birmingham Division of Preventative Medicine

As a post-doctoral scholar in the School of Medicine, Department of Medicine, Division of Preventive Medicine at the University of Alabama at Birmingham, I will be assisting on all data management functions including entry, verification, internal edit and logic checks for the COPD project. Assist in the analysis of the collected COPD project data, as well as secondary data analysis of other data sets. Additionally, I will collaborate with scientist on and off the UAB campus.

Capt Don Michael, MPH, CPH
U.S. Air Force Public Health Officer

I direct multi-disciplinary public health activities and personnel both at an Air Force base and in deployed settings. Activities/programs include communicable disease control and surveillance, occupational health/worker protection, deployment medicine/medical intelligence, medical entomology, food and public facility sanitation, preventive health assessments, and individual medical readiness oversight. My assignments have taken me to Ohio, California, Guam, Indonesia, and the Middle East. My career field focuses on the direct application of public health principles in support of force protection and humanitarian assistance.

I am assisting with transforming the ailing US health care system by deploying meaningful use of technology and electronic medical records. I steer organizations through scoping and project structuring exercises; I lead and facilitate work flow and content design meetings and workshops with clinicians, ancillary teams and other groups; advise them on Order Set build, Clinical Decision Support, Rules and Alerts etc. The primary focus of my work is connected community of health, better health care quality, patient safety, continuum of care, value based patient care, lower health care costs, evidence based medicine and patient empowerment.

My responsibilities for program evaluation include improving evaluation tools, monitoring data, and preparing evaluation reports.I am also an instructor for year 1 and 2 Rural Scholars on Public Health topics like Basics of Epidemiology, Screening, Research Design, and Cultural Sensitivity.
