Founders' Activity Center FAQs
Answer: To terminate your membership, simply fill out the Payroll Deduction form located under the "Membership Forms" section of our website and sign the "Termination" line. Once signed, send the completed form via campus mail to Megan S--FAC to finish processing. All forms must be received at the FAC no later than the 9th of each month to ensure that no deduction will be pulled on your next paycheck.
